You are asked to report each year no later than 18 months following the receipt of the award.

Please use the “Annual Follow-Up Form” provided at the right to produce your report.

You may add pages, if necessary. Then email it using the link to the right that says

“Email Follow-Up Reports Here.”

The Henry and Sylvia Yaschik Foundation awards grants to qualified charities that serve the greater Charleston,  South Carolina area.
